Our Placement Services for Residential Care Homes

West Valley Senior Placement Residential, operating under At Ease Care Centers, LLC, is dedicated to supporting residential care homes by connecting them with clients from At Ease Care Centers. Our services are designed to foster a straightforward and mutually beneficial partnership, as outlined in the Resident Placement Agreement (RPA) and Statement of Work (SOW) dated June 12, 2025.

Placement Process

Our structured approach ensures seamless client placements, aligning with the SOW’s referral services and the RPA’s placement terms:

  • Initial Consultation: We assess your care home’s capacity, services, and needs through a detailed intake form. This step ensures a tailored match, as required by the SOW’s pre-assessment process. You must confirm the resident’s physical and mental fitness before admission, per the RPA.
  • Client Matching: We identify and refer clients from At Ease Care Centers based on your availability, financial, health, location, and personal needs criteria. The SOW mandates this pre-assessment, and referrals are formalized through an RPA for each admitted resident.
  • Placement Support: We manage the transition, including follow-ups to confirm client satisfaction and address any issues. The SOW specifies ongoing support, with a dedicated point of contact available Monday to Friday, 9:00 AM - 5:00 PM MST.
Fee Structure

Our flexible payment options, detailed in the SOW and RPA, support small businesses:

  • Option 1: $1,250/month for 6 months ($7,500 total).
  • Option 2: $1,500/month for 4 months ($6,000 total).
  • Option 3: $2,000/month for 3 months ($6,000 total).
  • Option 4: $2,500/month for 2 months ($5,000 total).
  • Option 5: $4,500 one-time fee for a single client placement.
  • Payments are due as outlined in the RPA and SOW, with referrals pausing if balances are overdue until payments are current.
